Requests for our services are welcomed and may be made by the individual, the family, friends, clergy, physicians, other healthcare professionals, and community agencies. Call 416 499-2185 or e-mail us at info@evgcares.org

If you make a request on behalf of a friend or relative, they must give their permission for us to contact them about services.

Referrals from agencies or physicians can be faxed to 905-472-4128. You may either print the referral form or complete it online using the options below.

Due to the high demand and limited staff availability, there may be a waiting time until an opening becomes available.
