Living Room Day Program

A weekly opportunity to meet with other people diagnosed with a life-threatening illness and take time for themselves. Participate in conversation, information sessions, expressive art or music and enjoy a meal, or just be in a supportive environment.

Social Activities and Resources

Compassionate Communities

Evergreen’s Connection Cafes were created to connect people with similar interests to share activities and community resources.

We offer a Connection Café Club once a week. Each Club has a focused activity. We have a Games Galore Club, a Walking Club, a Book Discussion Club, and a Creative Club.

All Café Clubs help people who are looking for social activities and community events, in addition to providing interest-based activities to those who attend. They are open to clients, volunteers, staff, and community members – everyone! These clubs are not support groups – just a way to connect people.

Walking Club

The Evergreen Connection Café Walking Club will meet the 2nd Monday of each month from 10:00am to 12:00pm from May to October, weather permitting.

Walks will take part on the trails around Evergreen Hospice and be approximately 1-2 hours in length. We have done our best to ensure washroom access and easy trail options, with spots to rest, but please ensure that you are well enough to walk. We urge you to seek the advice of a physician before beginning any physical exercise activity.

Your participation in this club requires registration, an awareness of your fitness to participate, and an understanding that any physical activity carries a risk of injury. It is your responsibility to ensure you don’t exceed your limits and inform volunteer, staff, or other participants if you begin to feel unwell during the activity.

You will be required to give Evergreen an emergency contact.

Participants must be registered before joining the Walking Club. Registration is through tstanton@evgcares.org or 416-499-2185 ext. 107

Book Club

The Evergreen Connection Café Book Discussion Club will meet the third Friday of every month from 2:00pm to 3:30pm in person or on Zoom.

We will discuss books that focus on self-care, grief, and other healthcare related topics. There will not be a specific book for everyone to read, instead it will be more of a reference library for those interested in finding their next book and/or recommending and discussing one they have found to be helpful on their journey.

Death Cafes

Join others in a candid and open, virtual conversation (via zoom) on death and dying. This is open to everyone.

“At a Death Cafe people, often strangers, gather to eat cake, drink tea and discuss death. Our objective is to increase awareness of death with a view to helping people make the most of their (finite) lives’. A Death Café is a group directed discussion of death with no agenda, objectives or themes. It is a discussion group rather than a grief support or counselling session.” – Deathcafe.com

Creative Club

The Evergreen Connection Café Creative Club will meet the last Friday of every month from 1:00PM to 2:30PM in person or online, depending on the activity being offered.

Every month, we will offer a creative activity and discuss how creativity can be helpful to your wellbeing and self-care.

Participants must register before participating. Registration instructions will be listed with each Creative Club posting.

Advance Care Planning & Substitute Decision Making

During this event, we will determine:

1) Who your substitute decision maker is (everyone in Ontario has one)

2) How to ensure they understand your goals, values, and wishes so you feel they are able to speak for you in the event you are unable discuss what you need to know and understand if you are to speak on the behalf of loved ones as a substitute decision maker

We will also be having an Advance Care Planning Discussion Cafe on Tuesday, April 16, 2024 from 2 pm - 4 pm.

Come celebrate Advance Care Planning Day and play the Death Deck with us! Advance Care Planning involves having important conversations with our loved ones about wishes, values, and beliefs. Join us as we practice saying what is in our minds and in our hearts about our future health. We will explore conversation tools and play the game the Death Deck!

This session requires prior Advance Care Planning knowledge. If you have not had a chance to attend one of our Advance Care Planning Information Sessions, please visit the HPCO's Advance Care Planning website and work through the information provided there to prepare.
